We encourage you to complete all forms at your convenience prior to your appointment. This will allow you to provide the most accurate and thorough information, and it will save you time at check-in when you arrive.
With our mobile check-in, you will receive a link via text or email 5 days prior to your appointment. You can complete your registration from wherever you feel most comfortable and safe—at home, in your car or from another personal space. You will be able add/update your clinical and demographic information as well as take a photo of your current medical insurance and driver’s license to add to your medical record.
If you are unable to use a mobile device to check in, please arrive at least 15 minutes prior to your scheduled time to complete your paperwork. A team member will provide a tablet upon your arrival for you to complete your registration electronically.
