Registration
We look forward to meeting you at your appointment!
We encourage you to complete all forms at your convenience prior to your appointment. This will allow you to provide the most accurate and thorough information, and it will save you time at check-in when you arrive.
With our mobile check-in, you will receive a link via text or email 5 days prior to your appointment. You can complete your registration from wherever you feel most comfortable and safe—at home, in your car or from another personal space. You will be able add/update your clinical and demographic information as well as take a photo of your current medical insurance and driver’s license to add to your medical record.
If you are unable to use a mobile device to check in, please arrive at least 15 minutes prior to your scheduled time to complete your paperwork. A team member will provide a tablet upon your arrival for you to complete your registration electronically. Thank you!

For your convenience, we’ve made it possible for you to make your payments via several methods, whether you prefer to pay in person, by mail, by phone, or online through our Online Bill Pay service. Please note that you may receive separate bills from facilities, anesthesia, laboratories or other service providers. Those bills cannot be paid via our online bill payment service.

Pay in office
Our receptionists and billing coordinators are available Monday through Friday during regular office hours to accept your payment. We can accept cash, check or credit card transactions.
